Application Instructions

Complete the following steps in the order listed to ensure proper submission of your online application and application materials.

Email collexl@csun.edu if you experience any technical issues with your online application. A staff member will contact you to assist you (Monday – Friday, 8am – 5pm).

STEP 1

Submit online:

You will be given an opportunity to access, complete, and upload the following required documents as part of your online application:

  1. Online application (CLOSED)
  2. Personal Statement- The personal statement should be a minimum of three to five double spaced pages, describing:
    • Your previous professional experience.
    • Why you are interested in applying for admission to an MPH degree program in health education.
    • How completion of an MPH degree in health education will advance your professional goals.
  3. Resume
  4. Application fee - Pay $55 as a nonrefundable application fee

Complete and submit the online Application as soon as possible.  Doing so will assure that your application is included in the applicant pool and that Admissions is aware of your application and can track additional materials as they arrive.

STEP 2

Submit by Regular Mail:

  1. Submit three letters of recommendation (two letters from current or former professors and one letter from an employer is preferred).
  2. Official Transcripts – Please mail original or certified copies of all official academic records/transcripts from all academic institutions you have attended in the past, not just from institutions at which you earned a degree to the exact address below. (If you attended CSUN, you do not need to request CSUN transcripts. We will acquire your CSUN records for your application. No copies of transcripts are accepted. Transcripts must be originals. The applicant must contact each college or university attended and request official transcript.
  3. Original or certified copy of TOEFL, IELTS or PTE Academic score. Scores can either be electronically transmitted to CSUN's school code (4707) or official copies can be mailed to the address below (Required of international applicants who did not earn a bachelor's degree from a regionally accredited institution within the United States).

Mail all official transcripts and your recommendation letters together in one package to the exact address below.

All items sent by regular mail must be postmarked by the application deadline date.

Please note: While not a requirement of the application process, the admissions committee reserves the right to contact applicants to participate in an interview.

DO NOT mail items until AFTER you submit your online application. Otherwise, we will not have a record of you as an applicant and your transcripts and recommendations cannot be processed and evaluated.

DO NOT mail or electronically transmit official transcripts to CSUN’s Office of Admissions & Records. We cannot access transcripts sent to that office for up to three months. This will prevent us from being able to review your application in a timely manner and consider you for admission.

Applicants must submit authentic and official transcripts of all previous academic work attempted. Failure to file complete, accurate, and authentic application documents may result in denial of admission, cancellation of academic credit, suspension, or expulsion (Section 41301, Article 1.1, Title V, California Code of Regulations).
