Application Forms

Participants will need to complete two forms as they progress through the program. These forms are submitted at two different times:

• Before starting the program or no later than the second course: Certificate Program Application
• After completing all work: Study Plan

Certificate Program Application
Those interested in earning the CSUN Certificate of Advanced Professional Development must apply to the program by submitting a completed Certificate Program Application, accompanied by the non-refundable application fee.

Study Plan
Upon completion of all required work, qualified students obtain their certificates by submitting a completed Study Plan and all supporting documents.

Each of the above forms in PDF format uses 1-2 Mb of disk space. Download and install the free Acrobat reader program to view them.
